The skin surface outer layer, the stratum corneum (SC), is a formidable barrier inhibiting all light wavelengths from penetration into underlying tissues. The SC layer produces about 100 to 1000 times the light scattering coefficient compared to the viable tissue below the SC Layer. The stratified SC layer is a complicated inhomogeneous opaque lipid matrix containing remnants of dead cells, cell walls, keratin, cell organelles and water. Each SC Layer component has a different refractive index rendering the layer to be opaque and restrictive to therapeutic light.
The SC Layer light scattering confines and traps a substantial portion of applied light energy causing skin surface heating as well as lateral light scattering. Realized light energy flux reaching treatment targets in deeper tissues is related to the applied light power. Applied light power level is limited by skin surface heating.
In darker skin tones, SC Layer melanin particles differentiated from epidermal keratinocytes, is found in the form of ‘dust’ contained within each SC corneocyte. The amount of melanin dust is related to skin tone. Light scattering increases and light penetration decreases as skin tone darkens. Darker skin tones significantly inhibit therapeutic light penetration through the skin surface.
